Academic Information


Please confirm that you are currently enrolled in a four-year college or university for classes in the upcoming academic school year. 


Enrollment in a community college (2-year) program is inelligible for scholarship through this program.



Please provide contact information for the college/university's Financial Aid Office, including the contact's name, email, and phone number.



To expedite scholarship payment, please provide the address of the location and/or person where your college/university accepts scholarship payments (i.e. Financial Aid Office, Enrollment Office)


Please confirm with your school if there is a deadline date that your college/university stops accepting external scholarship submissions for the upcoming school year. If yes, please provide the date below.


 

Documentation


Please upload the following documentation (PDF required)

-Official Transcript (no screenshots or editable Word docs)

-The Award Letter detailing your financial aid package for the upcoming academic school year


Please confirm with your school if there are any forms that are required to complete in order to finalize your scholarship renewal. Also provide any additional information the FWA may need to process your scholarship renwal.


When documentation is not available at the time of application, you may send the award letter and transcript to [email protected]. All documents must be sent electronically and not by mail. All documentation must be received by July 8, 2024.
